Independent Provider

Self-Directed Care for Living at Home

If you would like to live at home, but need some assistance, the Independent Provider program may be of help to you. You will receive assistance from a Personal Care Aide, whom you select and supervise.  Your aide can support you with personal care and homemaker activities such as:

  • Bathing
  • Grooming
  • Dressing
  • Preparing food
  • Grocery shopping
  • Cleaning & laundry
  • Transportation
  • Medication reminders
  • Budgeting
  • Other personal care & medical-related assistance

 

You’ll determine:

  • Who you want in your home
  • The most convenient times for your schedule
  • How your Personal Care Aide can best support your goals

Our staff will help you initiate these services. Your budget for a personal care attendant will be determined based on your functional needs. An “Individual Service and Spending Plan” focusing on specific goals will be created.

Even though your Personal Care Aide is paid through Medicaid funds, you are considered the employer. As a consumer/employer, you have the right to hire, fire, schedule work, supervise and direct your aide.  You get to decide who will be your aide and what he or she helps you with based on the assessment of your needs. Assessments are performed every quarter to be sure your needs are being attended to properly.

How do you know if you are eligible?

To participate, you must:

  • Be 65 years or older, or an individual with a disability who is over 18 years old
  • Possess the ability to self-direct all aspects of your care, or have a representative who is available and willing to direct and manage your care.
  • Have a cognitive or physical disability that affects your ability to complete Activities of Daily Living in a safe or timely manner.  
  • Have been approved for Medicaid Long Term Services and Supports (LTSS) through the RI Department of Human Services.
  • Meet Medicaid (LTSS) requirements for high or highest level of care.

This program is similar to our Personal Choice Program and is designed for those who are medically stable.  We will help you make a determination of what program is the best fit for you.
