At AccessPoint’s Living Rite Center, our focus is to empower you to improve your health, wellbeing and and community participation. Our services for eligible adults with developmental disabilities and/or dementia, promote better health and better quality of life.

Through Living RIte you are able to access preventative and chronic health care management. We also offer a variety of related services to enhance your well-rounded life, an important support for your overall health.

Some of the goals we may work on together include:

  • Better coordination of your health care services
  • Support your home-based lifestyle in the least restrictive environment
  • Reduce unnecessary emergency room visits and/or hospitalizations
